What is hreflang, and why does a wrong language code send users to the wrong page?

Hreflang is the rel=alternate tag (or HTTP header) that tells Google which language and region version of a page to show each user. One wrong locale code, one missing self-reference, and Google stops trusting the cluster.

Hreflang is a signal, not a directive. Google uses the tags to pick the right URL from a cluster of localized pages for each searcher. The cluster must include a self-referencing tag on every page, every language-region code must follow the ISO 639-1 and ISO 3166-1 Alpha 2 spec, and return links must be present (if page A says page B is the German version, page B must say page A is the English version).The failure modes are well documented and easy to introduce. A developer types de-DE when the spec wants de-DE (correct), or de_DE with an underscore (wrong). A CMS adds hreflang tags to the localized pages but forgets the x-default fallback that catches unmatched users.
